Soft Tissue Grafts or “Gum Grafts”
Dental Implants

Soft tissue grafts, commonly known as "gum grafts," are a dental procedure designed to treat gum recession and improve the overall health and appearance of your gums. Gum recession occurs when the gum tissue surrounding the teeth pulls back, exposing the roots of the teeth. This condition can lead to a variety of dental problems, including sensitivity, an increased risk of tooth decay, and cosmetic concerns. Fortunately, soft tissue grafts can address these issues by restoring gum tissue, providing both functional and aesthetic benefits.

What Are Soft Tissue Grafts (Gum Grafts)?

Soft tissue grafts, or gum grafts, are surgical procedures in which tissue is taken from one part of the mouth (usually the roof of the mouth) and grafted onto areas where the gums have receded. The purpose of these grafts is to restore gum tissue that has been lost or damaged due to gum recession, offering protection to the exposed tooth roots and improving the appearance of the smile.

Gum recession is often caused by periodontal disease, poor oral hygiene, aggressive brushing, or other factors like smoking or genetic predisposition. When the gums recede, the tooth roots become exposed, making them more susceptible to sensitivity, decay, and even tooth loss. A gum graft can prevent further gum loss, reduce sensitivity, and enhance the overall health and aesthetics of your gums.

Why Are Soft Tissue Grafts Necessary?

There are several reasons why a dentist or periodontist might recommend a soft tissue graft procedure. The most common reasons include:

  • Gum Recession: Gum recession is the primary reason for soft tissue grafts. As the gums pull back from the teeth, they expose the tooth roots, which can lead to increased sensitivity, discomfort, and an unsightly appearance. In severe cases, the exposed roots may become vulnerable to decay and infection.
  • Tooth Sensitivity: Exposed tooth roots are more sensitive to temperature changes, hot and cold foods, and beverages. Soft tissue grafts help cover the exposed roots, reducing sensitivity and improving comfort.
  • Aesthetic Concerns: Gum recession can create an uneven gum line, which can affect the overall appearance of your smile. Soft tissue grafts restore the gum tissue and improve the balance and symmetry of the gum line, giving you a more youthful, aesthetically pleasing smile.
  • Protecting the Tooth Roots: When the gums recede, the tooth roots are exposed to plaque and bacteria. This increases the risk of tooth decay and root cavities. Soft tissue grafts provide a protective layer, helping to keep the tooth roots covered and reducing the risk of further damage.
  • Preventing Further Gum Recession: If gum recession is not treated, it can continue to progress, potentially leading to more severe oral health issues, including tooth mobility or loss. A gum graft can stop the recession process and protect the teeth and gums in the long term.

Types of Soft Tissue Grafts

There are several types of gum grafts, each with its own benefits and suitability depending on the patient’s condition. The most common types of soft tissue grafts are:

  • Connective Tissue Graft: The connective tissue graft is the most common type of gum graft. During this procedure, a small flap of skin is cut from the roof of the mouth (palate), and the tissue underneath the flap is used to cover the exposed tooth root. The flap is then stitched back into place, and the grafted tissue is sewn onto the gum line where the recession has occurred. This type of graft provides a good source of tissue and is effective at covering exposed tooth roots.
  • Free Gingival Graft: In this procedure, tissue is taken directly from the roof of the mouth (palate) without making a flap. This graft is typically used when there is a need to add more gum tissue in areas where the gum line is thin or lacks enough tissue. Unlike connective tissue grafts, free gingival grafts do not involve the use of tissue from underneath the flap, so the donor tissue is directly exposed.
  • Pedicle Graft: A pedicle graft involves taking tissue from an adjacent healthy gum and moving it to cover the exposed root area. This method is typically used when there is enough gum tissue near the affected area. The donor tissue remains attached to the original site, and a small portion is moved over to cover the receded gums. This type of graft allows for a more efficient healing process because the tissue remains partially connected.

The Gum Grafting Procedure

The gum grafting procedure is typically performed under local anesthesia to ensure that the patient remains comfortable throughout the treatment. The steps involved in the procedure are as follows:

  • Initial Consultation: Before the procedure, you will undergo an assessment by your dentist or periodontist. The dentist will examine your gums, take X-rays, and discuss your medical history. Based on this evaluation, the appropriate type of gum graft will be recommended.
  • Anesthesia: Local anesthesia will be applied to numb the area of the mouth where the graft will be performed. In some cases, sedation may be offered for patients who experience anxiety or discomfort.
  • Tissue Removal: Depending on the type of gum graft, tissue will be taken from the roof of the mouth or adjacent gums. For connective tissue grafts, a flap of gum tissue will be created, and the underlying tissue will be removed. For free gingival grafts, the tissue will be directly removed from the roof of the mouth.
  • Grafting: The donor tissue is carefully placed over the exposed tooth roots, and the gum tissue is sutured into place. The donor site is then closed with sutures to promote healing.
  • Recovery: After the procedure, you will be given detailed instructions on how to care for the grafted area. The recovery time typically takes a few weeks, with most patients experiencing minimal discomfort. Pain relievers and antibiotics may be prescribed to manage any pain and prevent infection.
